The Oregon E-Government Service Desk offers services and support for many applications. The forms in the drop menu of this page all result in a service desk ticket being created and using the appropriate form offered here will ensure that we have all the ingormation we need for a specific request type and more timely processing and completion of your request.

Be sure to check the form you select as many of them have special instructions and/or pre-requisites at the top of the form that must be followed in order for the request to be accepted and fulfilled.

Some common selections are:
Request for access to the Oregon Public Meetings Manager or Statewide Newsroom application
Request a new Website Author
Request a new TPE user
Request a website redirect

Here are more forms you may want to use. If you don't see one specific to your need, use the top selection, "Service Desk Ticket Form".

Enter a short, but pointed description of this request. Please avoid subject lines like, "Help!" or "Password reset". Something like "SharePoint password reset" or "Add user to Google Analytics" are great.

This is the type of service that this ticket refers to.

Please enter the name of the Single Point of Contact for the affected website. If you are the SPOC, simply enter 'self'.

Please enter the email address for the SPOC of the affected web site or service. If you are the SPOC, simply enter 'self'.

If multiple redirects are being requested, please use the worksheet referenced above to list all the desired and destination URLs and attach it to this request.

Please enter the URL (address) that you would like to use. This URL will redirect to the Destination URL identified in this form.

If the existing link starts with www then its a good idea to have the redirect include the www variant. New marketing URLs do not require www to be a part of the URL.

Please enter the URL (address) the visitor should be delivered to when they click the Desired URL link.
