Use the service desk form for App Engine Submissions Access to request that agency, board or commission users get access to App Engine Form Submissions.
Create a login
To create a login use the below links to create a staging/test account and/or a production account.

Production Login: https://appengine.egov.com/submissions/Account/Login
*If you are requesting both staging and production access, you will need to create a login for each environment (using the links above).
Select option “NIC ID Sign in / Sign up”.
Select on “Sign up now” to create your account.
Enter in your agency/work email address, and click “Send verification code”. This will send an code to your email, and you will be asked to provide the code before you can select “Create” at the bottom of the page to set up your account.
To submit the request
Navigate to the App Engine Submissions Access form at the E-Government Service Desk. This will create the service ticket.
The Service Desk will review the request and respond with any questions that are appropriate. Once all information and approvals have been gained, the E-Gov Service Desk will:
- complete the request
- make the account holder aware of the changes that have taken place
- notify the requester that the changes have taken place (no passwords will be shared)
- close the service ticket
At this point, new users will have access to the AppEngine Submission Viewer system. Changes to existing users accounts should show up in the system immediately once the change is made.
